*Please note, we do background checks on ALL adults living in the home and need full names (including middle initial) and complete date of birth. Also, please call your vet and give permission for MUTT LIFE to access your records*

Who will be the dog's primary caregiver?

Please include FULL NAME and DATE OF BIRTH of anyone over 18 years old who lives with you (Required for background check).

Please list ages of any children in the home.

Do you have a fenced in yard? if so how high and what kind?

Please describe a typical day for your dog. Be sure to include your plans for training, socialization, stimulation and outdoor activities.

Where will they be kept when you are not home?

How many hours will your dog be left alone each day?

Where will you keep your dog at night?

Please list all animals owned in the last 10 years (dogs, cats, birds, etc.) including names, ages, and breed for each and if currently living in home.
